Hawaii Virtual Care, LLC (“HVC,” “we,” “us,” or “our”) operates the website hawaiivirtualcare.com (the “Site”) and provides virtual intensive outpatient behavioral health services to residents of the State of Hawaii. This Privacy Policy describes how we collect, use, and protect information gathered through the Site. It applies to Site visitors and to individuals who submit information through our web forms. It does not replace, and should be read together with, our Notice of Privacy Practices, which governs protected health information (“PHI”) we maintain as a healthcare provider.

When you complete a contact, insurance-verification, or admissions-inquiry form, we collect the information you choose to submit, which may include your name, phone number, email address, island or location of residence, insurance information, and information about the services you are seeking. Because we are a behavioral health provider, information you submit about your treatment needs may be sensitive; we treat all inquiry information with the same care we apply to clinical records.

Information collected automatically. Like most websites, we and our service providers collect certain information automatically when you visit, including IP address, browser type, device identifiers, pages viewed, and referring pages. This is collected through cookies and similar technologies as described in Section 4.

If you become a client of Hawaii Virtual Care, your treatment records are protected health information governed by the Health Insurance Portability and Accountability Act (HIPAA) and, for substance use disorder treatment records, the federal confidentiality regulations at 42 C.F.R. Part 2. Those records are maintained in a secure, HIPAA-compliant electronic health record system — not on this website — and are never disclosed without your written consent except as permitted by law. 4. Cookies and Analytics

The Site uses cookies necessary for its operation and may use analytics cookies to understand aggregate Site usage. We never disclose the identity of any person as a treatment inquirer or client except as the law permits.

6. Data Security and Retention

We use administrative, technical, and physical safeguards appropriate to the sensitivity of the information we hold, including encryption in transit, access controls, and role-based permissions. We retain inquiry information only as long as needed for the purposes above or as required by law, and clinical records for the periods required by Hawaii law and federal regulation.

7. Your Choices and Rights

You may contact us at any time to access, correct, or request deletion of information you submitted through the Site, subject to legal record-retention requirements. 8. Children

Our services are provided to adults age eighteen and older. The Site is not directed to children under thirteen, and we do not knowingly collect personal information from children.
